A hapless nurse served up a bottle of urine with a patient's breakfast during a disastrous night shift, a tribunal heard.

Titus Ogunjinmi plonked the bottle next to the woman's morning meal and then failed to give her the proper medication, it is said. 

The nurse is accused of leaving a used incontinence pad on a chair in the woman's room, and not bothering to clean up two bags-worth of bloodied bandages and dirty linen from another patient's room. 

Ogunjinmi did not refer to patients by name, dubbing one simply the 'knee woman', and referring to another as 'David' even after he was told that was wrong, the Nursing and Midwifery Council was told. 

The agency nurse is facing a string of allegations of poor care and misconduct while working the night shift at Holly House Hospital, in Buckhurst Hill on October 6, 2011. 

'This case relates to providing inadequate care to five patients', said Frances Ibekwe, for the NMC.

'There were soiled bed sheets, open and soiled dressings, and also a dirty dressing trolley.' 

Ward Sister Sally Hall was first called to attend to a woman recently out of knee surgery who needed help getting to her bedpan. 

'The patient was upset, tearful, anxious, and hyperventilating', said Ms Hall. 

Ms Hall said she was accosted throughout the day with other alleged problems caused by Ogunjinmi. 

Ogunjinmi is also accused of waking a patient every two hours to be observed. 

Medical records were left in the wrong patient's room, it is said, and one woman, patient D, reported not receiving her medication at breakfast time, instead having to endure the urine bottle being placed next to her breakfast. 

Ms Hall told the tribunal Ogunjinmi did not record medication accurately after giving it, was vague about patients at handover to the day shift, and missed out some key information. 

Ogunjinmi, who is at the tribunal in central London, has admitted four charges of failing to administer medication, but denies the rest of the allegations.
